2025 Wellperks Wellness Incentive Deadline is September 30

Earn a cash incentive by completing your annual physical by September 30, 2025 AND submitting the Physical Completion Form.

Your annual wellness exam gives you and your doctor a complete picture of your health and what you need to do to be healthier. We understand the importance of this one action, and we want to make it easier for you to schedule and complete your annual wellness exam each year. Here’s how:

• Annual Wellness Exam: Using the Anthem medical plan, annual wellness exams are preventive care; therefore, the visit is completed at no cost to you.
• WellPerks Wellness Incentive: When you complete your annual wellness exam, you’re eligible to receive the WellPerks Incentive – $550 for single coverage and $1,100 for dependent coverage (when both you and your spouse complete the exam).
• Healthy Day Off: Get a PAID DAY OFF for the day you complete your annual wellness exam! Request the time off like you would any day off, except select “Wellness Visit” to use the PAID Healthy Day Off. Learn more at SteelPerks.com.
• And in case you’ve forgotten, here are a few important reminders:
• You are eligible for your annual physical every new plan year (beginning 1/1); you do not need to wait one year from your previous physical.
• Only adults need to complete the exam (if your spouse is covered on our plan, then they need to complete their annual physical, too).
